The Forms should be filled in Capital
Letters.
b.
Please respond to all the information sought.
c.
Additional sheets may be used, if necessary.
d.
Ensure that both the Forms are signed.

Photocopies
of certificates |

Photocopies
of certificates regarding date of birth and
educational qualifications shall be enclosed
with the Application Form.

Original
Certificates should not be sent.

Mode
of remittance |
|
The
payment for the program is to be remitted along
with the Application Form at the time of
enrollment. Payments should be made through A/c
payee crossed demand draft of any commercial
bank, in favor of "The Council for
Portfolio Management and Research", payable
at Hyderabad. The payment can also be remitted
through credit card by holders of VISA and
MasterCard. |

recent color photograph affixed).

Draft/Credit Card Merchant Slip (wherever
applicable) towards the payment.

Educational Qualifications/Mark Sheets.

are advised to take note of the validity date
specified on the Application Form and ensure
that their Forms reach the Admissions Officer or
